State and licensing bodies Arizona

The Arizona Department of Health Services’ Division of Licensing Services regulates and monitors health and child care facilities to ensure quality care and safety. They conduct inspections and investigations to enforce performance standards. SB 1203 removes the need for architectural plan reviews for health care institutions, but Life Safety Code inspections remain mandatory.

All health care institution license applications in Arizona require a $50 nonrefundable fee, as per A.R.S. § 36-405(B)(5). Applications must include updated forms, such as the Initial License Application for Residential Health Care Institutions (effective May 1, 2022) and other required documents. Incomplete applications will not be processed.

Provider training includes Basics of Residential Licensing for new operators on applications and inspections, Maintaining Compliance for licensed providers on rules and deficiencies, and Sustainable Plans of Correction focusing on creating effective POCs with practical exercises. These sessions help ensure smooth operation and adherence to regulatory standards.
